REFERRAL CRITERIA

 

 

 

The Housing Link will consider all referrals from homeless people aged 16 upwards.

 

The project will consider self-referrals, and referrals from all statutory and voluntary agencies.

 

Applicants must be homeless, potentially homeless or in housing need.  In order to determine a service user’s current housing situation an interview will be undertaken, prior to acceptance into the Project, with two Housing Support Workers.  

 

All referrals are considered on an individual basis, however the project reserves the right to refuse applications from any person who in their judgement would jeopardise the security of the project or the well being of the staff or residents.

 

The project is not able to consider referrals from current users of Class A drugs, convicted schedule one offenders or convicted arsonists, into its Castlecroft Scheme.  Such restrictions will not necessarily apply to the rest of the project.

 

Applicants should have the potential to live independently.  In order to develop this potential the service user must, as a part of their condition of residency, be willing to participate in an on-going resettlement programme with a dedicated Housing Support Worker.

 

Within one month of the date of moving into the Project the service user and his/her support worker will produce an individually tailored support plan that identifies the service user’s needs and aspirations.

 

As part of the on-going condition of residency the service user will meet on a regular basis with their support worker to review and revise their individual support package and personal needs.  

 

We would always give priority to referrals from Bury MBC area and from other areas when this would assist LA Departments to fulfil their legal obligations.

 

AGENCY PROCEDURE

 

 

 

Initial Enquiry.

 

An initial enquiry can be made by telephone although a referral form will need to be completed, prior to an interview being arranged.

 

Referral Forms

 

Referral forms can be obtained by ringing 0161-723-2040.  The form must be returned to the address on the form.  Referring agencies may copy the form and retain a stock.

 

Unless absolutely unavoidable please do not fax referral forms, either send in hard copy to:

On receipt of the referral form, either the agency or the applicant will be contacted to arrange an interview.

 

Following the interview, which will be conducted by two staff members, a decision will be made and the agency and applicant will be notified within five working days.
